Claim Form Login
If you received a Notice of Data Incident regarding the Data Incident from D.C. & Associates of Central Florida, P.A. (“DCACF”) on or around July 26, 2023 and you would like to receive a benefit from the Settlement, then you must fill out a Claim Form. Settlement Class Members can make a claim for one or more of the following:
Free credit monitoring services;
Up to $2,000.00 in reimbursement for unreimbursed costs and expenditures incurred in responding to notice of the Data Incident, including up to $80.00 in compensation for time spent responding to notice of the Data Incident;
Up to $5,000.00 in reimbursement of any other unreimbursed costs or expenditures that are fairly traceable to the Data Incident, including up to $240.00 in compensation for time spent remedying issues related to the Data Incident; and/or
An alternative cash payment of $50.00
If the Settlement is approved and you are found eligible for a payment, you may receive a check or electronic payment.

To start your online claim form, you must login with your Unique ID and PIN. You can find your Unique ID and PIN on your email or postcard notice. If you are unable to locate your ID & PIN, please contact us by emailing info@DCACFDataIncident.com. Please request your Unique ID & PIN by providing the Class Member’s full name and mailing address.

The deadline to submit a Claim Form online is October 15, 2025. If you are mailing your Claim Form, it must be mailed with a postmark date no later than October 15, 2025.
